Fixes and Updates

• Fixed an issue where reviewer assignments were not appearing in the Application view
• Fixed an issue where applications were being assigned twice to reviewers
• Fixed an issue where not all reviewers were appearing under the Manual Reviewer Assignments section
• Fixed an issue where some recommenders where seeing a 500 error when trying to attach a file 
• Fixed an issue where a few recommenders were not being allowed to see their completed recommendation task
